I, REDACTED, of REDACTED Hospital Medical Center had the opportunity to meet with the patient on REDACTED to evaluate needs for inpatient medical health services. Mr REDACTED told me that he has had history of mental health since the age of 22. He states he has had several inpatient treatment at different mental health facilities and he is usually compliant in taking the medication prescribed to him. However, he feels it does not help him. He says that he is tired of living and that sometimes he feels like killing himself. He admits to overdosing on heroin with the intent on ending his life. He stated he has no family support and has been homeless. He confirms he is still suicidal during this interview. SW discussed with him the options for treatment either as voluntary or involuntary to which he responded he did not care, but proceeded to say he would accept to receive inpatient mental health care. He voiced he would contract for safety and would participate in groups with our mental health team. During our conversation, he also said that he did not have a choice, as we would commit him involuntarily if he did not cooperate. SW explained to him that he indeed he had a choice and how his attitudes about the services would change the outcome. Mr REDACTED proceeded to say that he would then accept mental health services voluntarily. SW contacted Dr. REDACTED , REDACTED Hospital Medical Center psychiatrist to discuss above. Based on patients history of non compliance and his ambivalence about service, Dr. REDACTED is requesting this SW to contact CDMHP as patient is not a good candidate for voluntary placement as he is still at high risk for hurting himself.
